Joseph Brant Hospital is currently seeking a Social Worker with an interest and a passion for joining a new innovative Mental Health & Addictions program for Transitional Age Youth and Adults designed to provide rapid access to acute stabilizing interventions through group and individual therapy and to facilitate active warm transfer and navigation to subsequent needed services across the region. This hub and spoke model, Prioritizing Health through Acute Stabilization and Transition (PHAST), is a collaborative partnership between JBH, CMHA Halton, ROCK and ADAPT. PHAST will run out of the Brant Community Health Centre located at 1230 North Shore Blvd.

In Partnership with Clients, Families and Stakeholders Responsibilities Include:
- Mental Health & Addictions assessments, delivery of acute stabilizing interventions, evaluation of interventions and treatment
- Facilitate development and implementation of clients personalized wellness plans
- 1:1 Instrumental support based on needs
- Group therapy development, delivery and evaluation
- Crisis Support
- Family engagement, support and education
- Community engagement
- Interprofessional, inter-agency engagement, relationship building and collaboration
- Identifying next stage services and facilitating navigation and warm transfer to these services
- Completing statistical information in accordance with Ministry and Program requirements
- Participate in continuous quality improvement cycles

- Demonstrated ability to tolerate ambiguity, embrace change and champion innovative ideas
- Comfort working across and with diverse stakeholders and ministries
- Commitment to evidence informed and evidence generating practice
- Passion for solving problems yet to be defined
- Commitment to transforming the Mental Health & Addictions system
- Comfort and flexibility to perform duties in the locations across the care continuum where clients and families need them most
- Embrace Technological advances to better meet with needs of diverse populations
- Masters of Social Work (MSW) .
- A member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum three (3) years' experience in a mental health setting providing assessment, treatment, consultation, and education to a similar adult target population; experience in community-based/community outreach service delivery preferred
- Demonstrated strong clinical assessment and treatment skills (Group & individual)
